For the subtitle snippet, use this code:

Then, use a URL like this:
https://yourdomain.com/success/?title=WHATEVER&subtitle=SOMETHINGELSE
Just replace yourdomain.com with your actual domain, /success/ with your actual page slug, and WHATEVER & SOMETHINGELSE with the information you want to display on the screen.
